Collaboration with Families

For families, the therapeutic mentor will act as a liaison between the support team, your loved one, and you.

For therapists, the mentor will meet with clients in real world settings to put therapy homework into action and take case management out of the therapy room.

For teams, the mentor will provide support in transitioning from a higher level of care to a lower level of care while acting as the point of contact between the individual and their treatment team. We bridge any and all gaps.

 

Weekly Email Updates


Bridge The Gap Services provides parents, family members, and/or loved ones with weekly email updates. Each week, an email is sent to the client's treatment team and family members to update everyone involved. This includes what our client accomplished that week, client's goals and plans for the following week, and areas for improvement.


Family Support Group

Bridge The Gap Services offers a virtual biweekly support group for all family members of clients current and past. Family members are encouraged to participate in their own self-care and therapeutic support, working alongside their loved ones they worry so much about.

When clients seek help, the family unit is often struggling and stressed, feeling hopeless about their ability to have a “normal family life.” It is important for the family members to have a place to come, get support, guidance and education about codependency issues, setting boundaries, self-care and taking the focus off of their loved one who has become the focus and putting it back onto the family’s individual members. In our weekly support group family members will find a safe, confidential environment to talk, ask questions and get guidance. Family members who have been in the trenches in the past, and are now in a healthier place are welcome to come and provide feedback and support as a way of paying it forward and being of service.

The support group is held the 1st and 3rd Tuesdays of each month virtually via zoom. Please reach out for more information or to participate.

Collaboration With Therapists

At Bridge The Gap Services, we work collaboratively with therapists. Therapists often seek us out when their clients are having difficulty applying therapeutic homework in real life. We provide accountability, structure, and consistency throughout the week so that therapists are able to take case management out of the therapy room. This allows the client space in therapy to focus on deeper issues while simultaneously accomplishing their day to day goals with their therapeutic mentor. In each client's initial intake, we consult with their therapist before constructing the client's action plan.

Collaboration With Treatment Teams

At Bridge The Gap Services, we work collaboratively with treatment teams to successfully launch their clients back into daily life through facilitating in the process of discharge planning and providing ongoing support.

Treatment teams often seek us out when their clients are preparing to discharge from treatment and would benefit from continuing support. It can be challenging for individuals to transition from a secure and structured environment while attempting to properly navigate maintaining balance in their daily routines and prioritizing their recovery.

We provide individuals support in softening the transition from a higher level of care to a lower level of care while maintaining a fluid line of contact between treatment teams and clients.

Collaboration With School Counselors

This therapeutic mentoring is designed to support students who are having struggles with mental health afflictions (depression, anxiety, ADHD, etc), family challenges, addiction/substance abuse, etc. A therapeutic mentor works alongside an individual’s school counselor and/or therapist to help support them and hold them accountable to their self-care and school work outside of the office.

Working with an individual’s school counselor allows the therapeutic mentor to help them learn organizational and time management skills, catch up on academic work, and complete assignments in a timely manner to achieve academic and social success. This success increases their self-esteem and confidence. The therapeutic mentor's role aims to relieve parent’s of the stress and frustration elicited by trying to manage their children’s studying so that they are able to focus on healthy communication and family dynamics. At Bridge The Gap, we believe that collaborative care and individualized support is critical for teens in need of some extra help.
